isa 2006 blocks outgoing traffic with no rule specified

I've created a rule near the top, allowing all outgoing traffic from "All Users" from specified IP range to external network. However, connections are blocked sometimes for no apparent reason. For example, one attempt to ssh to an external server would fail, and another immediatelly after that - won't. to the same server! logging shows thoose failed connections as denied, but "rule" column is blank.

any suggestions?
