What are the restrictions for the name of an application when submitting to App Store ?

Hello,

I have just finished my application for iPhone. I would like to know if it is possible to use the word iPhone in the name of the application. For example, can I name my application iPhonesque ? And for the icon, can I use graphics related to Apple like a Mac Windows (with the three buttons, red, yellow and green) on snow leopard background ?
